**Please note: Asthma, Epi Pen, and Medication Forms are to be completed for each school year

REMINDER FROM THE NURSE FOR 2021- 2022 SCHOOL YEAR:

Epi pens -If your student is prescribed an Epi pen, please have the Epi pen packet completed by the physician and brought to the nurse along with an Epi pen the first day of school. Epi pen forms can be found on the website under Nurse.

Asthma -Asthma Action Plans/doctor orders for inhalers should be completed and turned in to the nurse by the first day of school. Forms can be found on the website under Nurse.

Seizures/Epilepsy -NJ passed a new law called Paul’s Law.  If your student has a history of Seizure Disorder or Epilepsy, which requires an Individual Emergency Health Care Plan for school, please have your child’s doctor complete a Seizure Action Plan for the school year.  ALSO, the school nurse shall develop an individual health care plan and individual emergency health care plan for the student, provided the parent annually provides the board written authorization for the provision of epilepsy or seizure disorder care.  An authorization form can be found on the website under Nurse.

Diabetes -All diabetic supplies/snacks and Diabetes Medical Management Plan completed by the family and doctor for the school year are due the first day of school. Please complete the Glucagon Delegate Authorization form as well, which can be found on the website under Nurse.

*ALL DOCTOR’S ORDERS/MEDICATION ORDERS NEED TO BE RENEWED EACH SCHOOL YEAR
